New Patient Registration  registration

To register with the Practice please just ask at reception for the appropriate forms and information. Alternatively, print and complete an NHS Registration Form in advance of attending the surgery.  You will be required to provide proof of your identity and evidence of your address.

You will be requested to complete some additional Practice patient registration forms and you may also be asked to arrange an appointment with one of the GPs to discuss your registration request. Please book your very first appointment with a doctor for 20 minutes. This gives you the opportunity to visit the Practice and to give us relevant information about your medical history.

Parents of children under the age of 5 will be contacted by our Health Visitor.

Nowadays, patients register with the Practice rather than a specific doctor. However, patients can express a preference to see a doctor of their choice. If this affects you, then please advise a member of the administrative staff when you are booking an appointment. We will make every effort to accommodate your request for a preferred doctor. However, it may not always be possible to do this due to holidays, emergencies etc.

Kinglass is a cross border Practice and whilst the vast majority of our patients come under the remit of NHS Forth Valley Health Board, a small proportion fall under NHS Lothian Health Board. The Practice accepts patients from Bo’ness and Blackness who fall under the umbrella of NHS Forth Valley, and Linlithgow patients who come under NHS Lothian.
