New Patient Registration  registration

To register with Kinglass Medical Practice you'll need to complete a registration form.

You can download this registration form, complete it and then email it to the Practice or hand it in.

You may need to download Adobe Acrobat Reader to edit and save your form.

We ask that you also provide us with evidence of your identity at your given address within our Practice boundary. We will always ask new patients so that we do not discriminate.  Not being able to provide us with this evidence will not prohibit a patient from registering with us. 

You may also be requested to complete some additional Practice patient registration questions such as whether you are a Carer for someone.  

Parents of children under the age of 5 will be contacted by our Health Visitor.

Nowadays, patients register with the Practice rather than a specific doctor. However, patients can express a preference to see a doctor of their choice. If this affects you, then please advise a member of the administrative staff when you are booking an appointment. We will make every effort to accommodate your request for a preferred doctor but this may not always be possible in the timescale that meets your needs due to holidays, emergencies etc.

Kinglass is a cross border Practice and whilst the vast majority of our patients come under the remit of NHS Forth Valley Health Board, a small proportion fall under NHS Lothian Health Board. The Practice accepts patients from Bo’ness and Blackness who fall under the umbrella of NHS Forth Valley, and Linlithgow patients who come under NHS Lothian.
